Topamine is approved for desensitisation. There is also a substantial body of evidence that 38% SDF is successful at managing carious lesions to arrest or prevent their progression. There is much greater evidence for a 38% formulation containing ammonia (eg Topamine) over non-ammonia products, or those with a different concentration. 
We are really excited to be able to offer this now to clinicians. It can be used for all patients and is a crucial part of prevention. 

There are multiple clinical situations that this can be used it. It is easy to apply. There are multiple possible application techniques. Our preferred technique is:

Standard PPE and apply vaseline on the lips
Isolate the affected area 
Dry the lesion or surface, remove any food debris
Dispense SDF. 1 drop can treat 4-5 teeth
1 drop per 10kg
Apply SDF to the lesion and allow it to be present for a minimum of 60seconds and up to 3minutes
Allow it to soak in for 60 seconds and up to 3 minutes for cooperative patients.
Remove excess with cotton rolls, gauze etc
You can choose to apply a fluoride varnish over the top. This can mask the taste and may 'retain' the SDF more. 

Precautions
  • Contraindicated for an allergy to silver or any components
  • Do not apply in the presence of ulcers
  • Avoid in exposed pulps or irreversible pulpitis
  • It will stain teeth. As such, ensure that patients / parents are aware if applied to visible surfaces
